PRACTICE POLICIES

New patients will be asked to sign and return a copy of the practice policies prior to entering treatment. The policies will be a part of the intake forms sent electronically prior to the appointment. Some of the highlights of the policies are as follows:

 

  • Allow up to 2 business days to return phone calls.
  • Avoid calling outside our normal office hours, unless it is an emergency. Always leave a message with your name and phone number.
  • If in a crisis, such as suicidal ideations with plan or intent or a life-threatening medical emergency, please call 911 or go to the nearest emergency. Please note that the suicide hotline number is 988.
  • Make every attempt to address your concerns during appointments. Phone conversations with the physician will be charged a fee which may or may not be covered by insurance.
  • Call at least 2 business days ahead of time to cancel or reschedule an appointment. Cancellations within 2 business days of the appointment, including now shows will be charged a no-show fee of $50.
  • A total of 3 no-shows is grounds for termination from the practice. 
  • Disrespectful behavior, intimidation or verbal or physical aggression towards the providers or staff will not be tolerated and is grounds for immediate termination of care.

REFILL POLICY

  • It is the patient's responsibility to ensure that medications are refilled at time of appointments. 
  • Medication refills are usually approved during appointments to last until the next appointment.
  • A fee of $20 applies for refills outside of appointments. 
  • Allow 1 to 2 business days to process the refill requests.
  • We do not honor medication refill requests from pharmacies either via phone, fax, or electronically.
  • Refill requests on weekends, starting Friday’s at 4:00pm, will not be accepted until the following Monday.

PAYMENT FOR SERVICES RENDERED

  • Payments for service are due at the time of appointment prior to being seen.
  • A credit card must be kept on file (stored safely in the electronic health record system) during treatment at CBHSD.
  • Payments will be billed to the credit card held on file unless other arrangements are made at the time of service.
  • The fees for services rendered is not refundable. Any demands asking for refunds based on disagreements regarding diagnosis or treatment will be considered inappropriate and will not be entertained. 

CONTROLLED SUBSTANCES

  • By law, OARRS will be checked prior to prescribing any controlled substances. 
  • Medication must be used as directed. Failure to do so will result in termination from the practice.
  • Please note: controlled medications will not be prescribed on the first visit.
  • Prescriptions of controlled substances is completely under the discretion of your physician/provider.
  • Please note that we do not continue controlled medications prescribed by providers from a different practice or organization.
